Effects of TOPIMAX on other antiepileptic drugs The addition of TOPIMAX to carbamazepine, valproic acid or lamotrigine has no or little effect on their steady-state plasma concentrations. In occasional patients, treatment with topiramate and phenytoin may result in an increase of plasma concentrations of phenytoin. Effect of other antiepileptic drugs on TOPIMAX At simultaneous treatment with phenytoin or carbamazepine the plasma concentration of topiramate decreases, probably due to induced metabolism. The addition or withdrawal of phenytoin or carbamazepine to TOPIMAX therapy may require an adjustment in dosage of the latter. The addition or withdrawal of valproic acid or lamotrigine does not produce clinically significant changes in plasma concentrations of topiramate. Concomitant administration of topiramate and valproic acid has been associated with hyperammonemia.
