That does not belong in either category. One authority refers to it as unique sedativeeuphoriant-psychedelic drug."9 Researchers in the early 1990s discovered and isolated cannabinoid receptors in several key areas of the brain responsible for movement and posture, control of the senses, concentration and memory. Cannabinoid receptors have also been located in components of the immune system, the heart, lungs, endocrine system and reproductive organs.10 Products containing cannabis include marijuana, hash and hash oil, which are derived from the plant Cannabis sativa. For more information on the various drug categories, see the Table 2 on the next page, which also outlines two other categories of drugs: antidepressants and anabolic steroids, for example, paracetamol.

I would protest against the usurpation on the part of these men [purveyors of pharmaceutical] of our function as teacher. GERD treatment is dominated by proton pump inhibitors PPIs ; for pharmacological control of symptoms and inflammation. It would seem unrealistic to expect any other class of drugs to be superior to PPIs due to their efficacy, safety, and broad utility. However, in recent years, evidence has accrued that a significant number of patients continue to have symptoms despite PPI therapy.105 In some cases, this may simply be a matter of too low a dose of the PPI. Nevertheless, acid reflux cannot explain residual symptoms in many GERD patients on PPI therapy, and the role of non-acid reflux has come into focus. In this context it should be noted that PPIs have an inhibitory effect on gastric volume secretion, which may explain why they also inhibit non-acid reflux as shown by a reduction in bile101 and possibly also other forms of non-acid reflux. Prokinetics such as 5-HT4 agonists cisapride, mosapride, and tegaserod have proved to have limited therapeutic utility in GERD, and even powerful gastrokinetics such as motilides appear not to have any major effect on reflux and symptoms. The concept of reflux inhibition holds more promise at this early stage, but the effectiveness of such agents must be demonstrated in large studies. The issue of tolerability and safety is even more important. Unfortunately, the prospect of discovering a target selectively involved in the TLESR pathway similar to the role of the gastric H, K-ATPase in the acid secretion pathway ; appears fairly weak. All known reflux inhibitors to date act on neuronal targets and may therefore entail associated side effects. Whether these would be acceptable in the treatment of GERD has to be taken into account at an early stage of development. Reflux inhibitors may have a role to play in treating extraesophageal symptoms of GERD such as coughing and asthma. Airway responsiveness to even slightly acidic aspirated fluids is very high, so effective control of intragastric pH may be insufficient. Pediatric reflux is another field in which there is a high unmet medical need, particularly in neonates. Here, acid plays very little role, and volume regurgitation through TLESRs is the dominating cause of symptoms such as failure to thrive and aspiration. However, the potentially highest utility of reflux inhibitors may be in combination with PPIs in PPI partial responders. With time, the exact role of reflux inhibition as a novel therapeutic modality in GERD will materialize, but much work remains in defining eligible patient populations and designing clinical studies tailored for these groups. Further, translation of the degree of inhibition of TLESRs into symptomatic improvement must be done. A close collaboration between clinical academic centers and the pharmaceutical industry is a key component in accomplishing this goal and zelnorm.

Liver transplantation requires exceptional surgical expertise and institutional support. Therefore, for many years, it was possible only in centres in the West with history of excellence in medicine and commitment in research. The acceptance of liver transplantation in Asian countries has been slow, chiefly due to vastly different cultural, ethical and religious backgrounds with different expectations from medical therapies in the context of developing economies. These conditions are certainly not conducive to serious research efforts. Unlike Japan, Hong Kong accepts brain stem death as practised in the west, but in spite of this, voluntary donors from consenting families have not been forthcoming in our conservative Confucius ethical milieu. While Hong Kong physicians generally condemn organ trading, the consensus has been more for organ donation on a voluntary basis, rather than adopting the Singapore 'opt-out' model. There is much to be desired in this rather passive approach with a population of over six million people and with rising expectations in a city known for its economic resources and media might. Under the Government of Hong Kong and now more directly the Hospital Authority, who decides on the distribution of the already very stretched medical resources for the pressing clinical services, and in the absence of a strong organ sharing structure locally among hospitals, it is indeed no small achievement for the two teaching hospitals in Hong Kong to start liver transplant programmes independently in 1991. Expressed concerns about long-term effectiveness and safety of ADHD medications 30 ; . Widespread failure to take medication properly and persistently has been suggested as a possible explanation for lack of evidence of long-term effectiveness of medication therapy 31 ; . This study supports the legitimacy of that claim and underlines the need to improve understanding of nonadherence and of measures aimed at improving adherence, especially among girls. At the same time, the low prevalence of long-term users observed in our study allows us to conclude that relatively few children, perhaps only one-fifth of those exposed to stimulation medications, would be at potential risk were long-term adverse effects ever identified.

12. Morad Y, Lemberg H, Yofe N, Dagan Y., Pupillography as an objective indicator of fatigue., Curr Eye Res. 2000 Jul; 21 1 ; : 535-42. PURPOSE: To determine whether parameters calculated from pupillary activity can identify subjects with sleep deprivation, and whether the objective values correlate with a subjective feeling of fatigue. METHODS: pupil size in the dark was recorded continuously for 10 minutes in 12 healthy volunteers using an infrared video camera. Two recordings were made for each subject: after a full night's sleep, and after 24 hours of sleep deprivation. Several parameters calculated from pupil size and activity were analyzed and compared with a subjective rating of the state of alertness provided by the participants in each test. RESULTS: All pupillary parameters differed significantly between alertness and fatigue p 0.0076-0. 0186 ; . Changes in one of the parameters - average pupillary diameter - correlated with changes in the subjective level of sleepiness r -0.51, p 0.028 ; . Although the values of most parameters differed among subjects, an absolute value of more than 25 in one parameter, cumulative pupillary variability ratio, was always associated with sleep deprivation. CONCLUSION: On-line analysis of the pupillogram using the suggested parameters can be performed easily to produce a real-time assessment of an individual's state of alertness or fatigue that correlates with his her subjective assessment of this state and tinidazole.

Absorption of zinc from soy protein meals in humans, " Sandstrm B., Kivist B., Cederblad A., J Nutr 1987 Feb 117: 2 321-7. -- absorption of zinc from soy protein-containing meals was studied in healthy human subjects by means of extrinsic labelling with 65Zn and whole body counting. Three types of soy protein, a soy flour, a soy concentrate and a soy isolate, were used in two types of meals: one consisting of rice and meat sauce and the other of white or whole-meal bread. Thirty and 100% of the protein of the meat sauce and 50% of the protein of bread was replaced with the soy protein products. Replacement of 30% of the protein in meat sauce had a marginal effect on the percentage absorption of zinc, whereas the absolute amount of zinc absorbed was lower due to lower zinc content in soy products than in meat. -- total replacement of the meat protein with soy ; , a significantly lower percentage and absolute absorption of zinc was observed. -- A lower absorption of zinc was observed when soy protein was added to white bread. The absorption of zinc from the whole-meal bread was low and did not change when 50% of the flour protein was replaced by soy protein. -- We conclude that the effect of soy protein on zinc absorption depends on the degree of replacement, the phytic acid and zinc content of the soy product and the protein content of the meal.

Performance was achieved. A competitively awarded contract was issued within thirty days and actual training began shortly thereafter. Next, Lieutenant General Walters commented on the changing nature of security cooperation. He stated that DSCA, in addition to traditional security cooperation, is now involved in managing several humanitarian assistance programs in association with the Agency for International Development. Examples of humanitarian outreaches include daily ration food distribution, wheelchair collection and redistribution activities, school construction projects, seaport repair and medical facility outfitting. The DSCA Director cited the singularly most prominent security cooperation accomplishment over the past year centered on Poland's selection of the F-16 aircraft through their international fighter competition. The U.S. security assistance community in conjunction with U.S. industry expended considerable effort to prepare and support the offer of F-16 aircraft. As part of this offer, the U.S. government approved a $3.8 billion loan to support the sale. This was the first loan of this type granted by the U.S. government since 1998. In addition to supporting Poland's national defense and enhancing their role in North Atlantic Treaty Organization NATO ; , the selection of the F-16 package opens the door for significant U.S. and Poland military-to-military interactions in the years ahead. On the horizon, the security cooperation community is preparing to support another key international fighter competition initiated by the Czech Republic. Breaking new ground, the U.S. government will not only submit its own offer of F-16A B aircraft but will also provide logistics and training support packages for the competing offers being tendered by the governments of the Netherlands, Belgium and Canada. This serves as another example of U.S. effort to work with other governments in areas of mutual security cooperation interest. Foreign military sales FMS ; in fiscal year 2003 was very healthy with sales of approximately $13 billion. Looking to fiscal year 2004, FMS sales are projected to reach $13.8 billion based on already approved or in-process congressional notifications. Regarding the topic of congressional notification, Lieutenant General Walters noted that the Congress gave careful scrutiny to a number of sales requests in 2003. The security cooperation community should expect Congressional interest in this business to continue in 2004. Lieutenant General Walters briefly reviewed some other security cooperation accomplishments. In early October 2003, DSCA released the updated version of the Security Assistance Management Manual SAMM ; which is available at dsca l. DSCA anticipates that the revised SAMM will prove to be a document that is easier to use and one that clearly articulates U.S. security cooperation policy. DSCA continues to work with activity based costing and management processes to better understand where security cooperation costs are generated and too better allocate FMS administrative funds. In this avenue, DSCA generated its first security assistance Program Objective Memorandum in an effort to improve security cooperation requirements identification and resource allocation. In regard to FMS cases, the number of open supply complete cases has been reduced by nearly two-thirds over the past three years. Case closure progress continues to improve under the Accelerated Case Closure procedures. In the information technology arena, the Security Cooperation Information Portal SCIP ; has debuted. SCIP provides customers a consolidated view of their security cooperation information as a short term vehicle pending the eventual fielding of the Case Execution Management Information System CEMIS ; . So far, approximately thirty customers have individuals registered as SCIP users. Work will continue in fiscal year 2004 to rehost key legacy security cooperation systems such as SAMIS, CISIL and MISIL ; into a common computer language. This action provides the basis and opportunity to modernize automated security cooperation processes and eventually transition to the entire security cooperation community to a single security cooperation execution management system, for example, buy zelnorm.

Competition Impower faces significant competition from both traditional and on-line advertising and direct marketing companies, which is likely to increase over time as additional capital, and companies enter the space. Traditional direct marketers pose less of a competitive threat in the Internet arena as most are locked into "old ways" of doing business and lack the experience or understanding to migrate to the Internet. Moreover, Internet direct marketing companies, which include online advertisers, e-mail marketers and other e-marketers, typically lack the direct marketing expertise to fully exploit the potential opportunities on the Internet and to do so profitably. Many are narrowly focused, media or technology oriented, and geared to selling the advertising inventory on their networks and or their proprietary e-mail data lists. Impower has a highly differentiated approach to Internet direct marketing and enjoys several competitive advantages, including: a management team with extensive experience in direct marketing; clients that include many of the world's leading Internet publishers, marketers and etailers; a marketer orientation, as opposed to the technology or media focus of its competitors. Impower partners with its clients to acquire customers and an allowable acquisition cost and maximizing the lifetime value of such customers; a 100% opt-in strategy, whereby it markets to the complete universe of 30 million opt-in email addresses, including client e-mail lists, Impower proprietary e-mail lists as well as third party e-mail lists; a strategy that focuses on utilizing a multitude of Internet direct marketing products and services to achieve the objectives of its clients, including products and services offered by third-parties and competitors; a transaction-based model whereby clients can pay for performance, thereby costeffectively acquiring and retaining customers; incentive programs and proprietary offers that motivate consumers to participate, while respecting their privacy; substantial expertise in data capture, data management, data analysis and data marketing; a pricing strategy based on the client's allowable customer acquisition costs, hence translating into real revenues with attractive margins; and an attractive business model with significant scalability.

In an openlabel study in the UK aimed at comparing the effects of withdrawing 6egaserod with continuous treatment, re-introduction of teegaserod in patients who relapsed after withdrawal provided symptom relief similar to the initial treatment.67 A German open-label study assessed whether retreatment with tegaserod in patients who experienced symptom recurrence following treatment withdrawal was as efficacious as the initial 12-week treatment period.68 A total of 513 patients entered the initial treatment period and 436 patients 85% ; responded to tegaserod 6mg twice-daily. During an eight-week follow-up after withdrawal, retreatment for four weeks with tegaserod was started in 307 patients with symptom relapse, with a response in 274 patients 89.3% ; . These open-label data suggest that IBS-C can be successfully treated and retreated with tegaserod. However, confirmation of these findings in a placebocontrolled study is necessary in line with the CHMP's recommendation to use a re-randomisation design and two primary outcome variables for drugs intended for short-term symptomatic relief of IBS.66 An international prospective, double-blind randomised placebo-controlled retreatment trial was conducted in women with IBS-C.69 Patients either received tegaserod 6mg twice-daily or placebo for one month. Patients with at least a partial response entered a treatment-free interval. Upon symptom recurrence, tegaserod-treated patients were re-randomised to tegaserod or placebo for an additional month. Primary efficacy variables were response overall IBS symptoms and abdominal discomfort pain ; to first and repeated treatment. Secondary outcome variables were relief of individual IBS symptoms abdominal discomfort pain, bloating and constipation; stool frequency and and ativan!

By Karen Witte Most of what goes on in your body occurs without your help or awareness: Your heart beats, your lungs take in oxygen and your digestive tract turns food into energy. You probably don't think much about any of this until something goes wrong and your body's automatic processes suddenly stop working. A case in point is gastroparesis, a condition in which the muscles in the wall of your stomach don't function normally. Ordinarily, strong muscular contractions propel food on its long and convoluted journey through your digestive tract. But in gastroparesis, the muscles in the wall of your stomach work poorly or not at all, preventing your stomach from emptying properly. This can interfere with digestion, cause nausea and vomiting, and play havoc with blood sugar levels and nutrition. Gastroparesis is a chronic condition, which means it is always present. A positive diagnosis is usually made with a gastric emptying study. This test measures the amount of time it takes for the stomach to move food through the stomach and into the intestines. No available treatment can cure gastroparesis. Dietary changes and certain medications sometimes help control symptoms of gastroparesis, but they're not effective in every case. uid fats, such as those found in whole milk and milk shakes however may be well tolerated and make a good source of much-needed calories. High fiber foods may aggravate gastroparesis in the same way as high fat foods. Since fiber tends to stay in the stomach longer, foods that are low in fiber should be included. Fiber is primarily present in whole grain breads and cereals, fresh fruits and vegetables and beans, peas, lentils, nuts, seeds and legumes. Therefore it is recommended to limit whole grains, and to avoid fruits and vegetables in their raw state. Vegetables should be well cooked until soft. Because gastroparesis interferes with digestion, people with the disorder may be deficient in important nutrients, including vitamin B-12, iron and calcium. A liquid vitamin and mineral supplement can help supply missing nutrients, but shouldn't be used as a substitute for meals. The vomiting caused by gastroparesis can lead to dehydration, so it's especially important to drink plenty of water. Taking small sips or sucking on ice chips may make it easier to keep water down when you're nauseous. Most people with gastroparesis do well eating pureed foods and using nutritional supplements. But some people with severe stomach problems may not be able to tolerate any food or liquids. In that case, doctors may place a feeding tube in the small intestine. The tube can be inserted directly into your small intestine through your skin, although a temporary nasal or oral tube -- the tube is threaded into the small intestine through your nose or mouth -- is often tried first to make sure you can tolerate this type of feeding. The tube is only used when gastroparesis is severe or when blood sugar levels can't be controlled by any other method. It lowers the pressure threshold for the peristalsis reflex, it increases the strength and frequency of muscle contractions, and it relaxes the pyloric valve that releases food from the stomach into the small intestine. Reglan also acts on the part of the brain that controls nausea and vomiting, helping relieve these symptoms in some people. But for all this, reglan has serious drawbacks, including side effects such as agitation, depression and severe muscle twitching, and also seizure or seizure-like activity in those prone to such activity. It's not intended for longterm use. Domperidone Motilium, is a drug thats similar to reglan but without many of its side effects, it's available in Canada and Europe. It is currently under review by the FDA for distribution in the U.S. Erythromycin. This drug isn't a prokinetic; instead, it's a common antibiotic that produces short bursts of strong stomach contractions. The drawbacks: Most people develop a tolerance to the drug fairly quickly; it can cause nausea, vomiting and, in some cases, hearing loss; and it may have significant drug interactions. For these reasons, erythromycin is usually used only intermittently or when symptoms become worse. Cisapride Propulsid monly used to treat severe cases of gastroesophageal reflux, cisapride has been linked to fatal heart arrhythmias and is not appropriate for anyone with heart disease or kidney problems. Cisapride also has the potential to cause serious drug interactions. It is only available under restricted access. Tegxserod Zelnorm.
