The aim of this report is to describe the changes in bone density associated with spinal brucellosis. Tipically, radiology shows destructive lesions with narrowing of the disc space, bony sclerosis and characteristically there is new bony formation with exuberant osteophyte. We reported 2 patients with spinal brucellosis, in whom the diagnosis were suspected by clinical and radiological findings, and confirmed by positive serological tests. Both patients were treated with doxicicline 100 mg BID plus rifampicine 600 mg daily for 6 weeks. Before and after treatment, dual X-ray absortiometry DXA ; scans were obtained using GE Medical System Lunar DPX-MD plus ; in both patients. Bone mineral density values in patient 1 change in L1-L4 from 1, 307 g cm2 Score T 1, ; to 1, 264 g cm2 Score T 0, 7 ; , but in vertebrae with spurs and new bony formation, bone loss after treatment were more pronounced. In patient 2, bone loss after treatment was found in the lateral spine scan, and change in B2-B3 from 0, 475 g cm2 Score T -2, 1 ; to 0, 361 g cm2 Score -3, 2 ; after treatment. Spinal brucellosis is associated with spurs and new bone formation. One explanation to the bone loss after treatment could be associated with a reduction in new bone formation.
