Discussion In this study, a physicianfocused educational intervention that included feedback to primary care physicians appeared to improve physic ia ns' pre sc ribing practices and blood pressure control in a sample of diabetic and nondiabetic treated hypertensive patients enrolled in a large physician network-based HMO. This improvement occurred based on a blood pressure goal of 140 90 mm Hg for the entire sample and persisted when the blood pressure goal was defined as 140 90 mm Hg for nondiabetic subjects and 130 85 mm Hg for diabetic subjects. There is currently an increasing focus on the role of primary care physicians in improving health outcomes through the development of systems that are able to help physicians identify, monitor, and proactively care for populations of patients with chronic conditions such as hypertension.12-13 A straightforward four-step process has been described to help family physicians design and implement population-based health care systems in their practices.13 These four steps include 1 ; selecting a condition for which evidence-based clinical guidelines are available, and outcomes are measurable, reliable, and relevant, 2 ; developing a mechanism to identify all patients within the total population who have the condition, 3 ; identifying measurable outcomes based on evidence-based care guidelines, and 4 ; continuously measuring outcomes to determine if there is improvement.13 This approach has demonstrated and arimidex.

Were collected on 40 patients. Some important lessons were learned about the use of the drug in clinical practice in Glasgow. Lesson1: A clinical protocol cannot be expected to answer academic questions that could not be answered by research studies. The question on predicting responders was not answered. Complete data were obtained for 28 of the 40 patients. The ability of individual clinical teams to operate the protocol and the perceptions of the usefulness of the project proved to be variable. However, for those 28 patients, the protocol seemed to be of value. Compared to the research data, it appeared that the benefit for the entire population prescribed the drug was similar to the oft quoted six months' worth of improvement. However, non-responders were identified by the protocol and the drug was usually only continued in those who showed significant response. Thus, health gain for the responders was three times greater than the health gain for the population prescribed the drug. Such information was not evident from randomised controlled trials and has been helpful in assessing the cost benefit ratio of the drug. Lesson 3: There is merit in allowing and encouraging specialists to develop, administer and validate novel ways of assessing benefit of new treatments. The `Gold Standard' for rating efficacy is a detailed cognitive test, the ADAS-cog. This was performed at baseline and after three months of treatment with Donepezil. In addition, a variation of the validated Informant Questionnaire for Cognitive Decline in the Elderly was administered. This was adapted to examine change in functional level during the first three months of treatment. It may turn out to be more useful than the ADAS-cog, for it is less time consuming and measures benefit in day-to-day function, rather than performance on abstract cognitive tests. This would streamline the assessment process and may allow for the eventual roll out of the protocol to Primary Care. Lesson 4: If clinicians are expected to return clinical effectiveness data, resources should be made available to assist with data collection. Although funding was made available for the cost of the drug, no funding for the evaluation was provided. Therefore, data collection had to be managed from existing resources within the budget for Elderly Mental Health Services. This has implications if the programme were to be significantly expanded. In conclusion, the Glasgow Cognitive Enhancement Protocol identified responders and has given valuable information on how best to deploy an exciting new treatment for dementia. It has identified a promising new tool for easier and quicker assessment. The information collected provided a useful addition to the information available from clinical trials. A 25-week randomized controlled trial of eptastigmine 12mg t.i .d . ; 10mg t.i .d . ; placebo ; in 342 AD patients, who had MMSE scores bewtween 10-26, was reported to yield better outcome as measured by the ADAScog p 0 .047 ; and CDR-S13 p 0 .010 ; . However, there was no difference in secondary endpoints which included IADL 2 . In another randomized controlled trial by the same Eptastigmine Study Group ; of eptastigmine n 491 ; MMSE scores between 10-22 ; , the investigators used a higher dosage 20mg t .i .d . ; 15mg t.i .d . ; placebo ; than the previous study. After 24 weeks of treatment, the high dose eptastigmine group 20mg t.i .d . ; showed a significantly better response in ADAS-cog, CIBICPlus and IADL scores than the placebo group 3. Cholinergic side effects were reported with similar frequency in both the eptastigmine and placebo groups . However, a dose-dependent neutropenia occurred with the use of eptastigmine . This adverse effect limited its clinical application. Overall, its efficacy is similar to other AChEIs . In view of its haematological adverse effect, it is better to avoid using eptastigmine note: not available in HK ; in patients . At present, one can use either rivastigmine or donepezil. New disease modifying treatments for dementia are greatly needed . In previous observational studies, estrogen replacement therapy has been found to be associated with a decreased risk of AD.
Cable to youth with OCD seen in clinical practice. Duke University and the University of Pennsylvania are noted for their expertise in the use of CBT for pediatric OCD. Despite a prestudy assumption that equivalent expertise would translate to equivalent outcomes, we identified a statistically significant site treatment interaction that indicates that the impact of CBT without concurrent medication was greater at the Penn site than at the Duke site, whereas no site effect was found for the combined treatment. Although this study used procedures designed to maximize protocol adherence, including direct supervision, case conferences, training meetings, and tape review, 12 these results nonetheless may have arisen due to a site or a therapist effect associated with alliance, competence, or protocol adherence ; , or perhaps to patient characteristics that may differ across therapists even though there were no apparent baseline differences in patient characteristics between the Penn and Duke sites. Because these variables vary in vivo, the presence of site differences can be thought to contribute to the generalizability of the overall result; eg, the overall outcome favoring CBT either alone or in combination with an SSRI cannot simply be the result of choosing sites with CBT expertise. Additionally, the strength of CBT alone at the Penn site contributed to our recommendation that CBT alone be a first-line option as initial treatment. Lastly, the finding that the sites did not differ in the impact of combined treatment suggests that when the results of CBT are attenuated for some reason, the addition of medication is important.
