Some antibiotics impair fermentative activities in the colon and reduce faecal SCFA levels. In the studies of HOVERSTAD et al. [183, 184] in which groups of six healthy volunteers were given antibiotics by mouth for six days, the effects on faecal SCFA were closely related to the amounts of antibiotic which were detectable in faeces. Reductions in total SCFA mmol kg faeces ; were seen with clindamycin 62.9 to 7.3 ; , bacitracin 105.4 to 21.8 ; and vancomycin 69.3 to 19.4 ; . Less striking changes were seen with ampicillin 62.4 to 47.8 ; and erythromycin 116.9 to 76.0 ; , whilst metronidazole, co-trimoxazole, doxycycline, nalidixic acid and ofloxacin had no effect. High concentrations of clindamycin, bacitracin, erythromycin, vancomycin and nalidixic acid were found in faeces. Molar ratios of butyrate decreased with clindamycin, ampicillin, bacitracin and vancomycin. Studies in vitro show that clindamycin, erythromycin and dicloxacillin reduce fermentative activity whereas penicillin and pivampicillin have no effect [185187]. Lactate does not accumulate in vitro indicating a general suppression of fermentation rather than specific bacteria being affected. However, a pronounced increase in succinate concentrations in faeces 4.2 to 40.0 ; in healthy subjects taking a combination of neomycin, bacitracin, colistin and nystatin [188], together with reduction in SCFA 77.8 to 19.0 ; , was seen in the studies of WILSON et al. [189] suggesting inhibition of succinate utilising species. Disease: What does it look like? Antibiotics are important for treating overt secondary bacterial infection. Staph aureus is the primary organism implicated in infected eczema. Oral antibiotics and antibacterial antiseptics may be helpful in controlling eczema flare ups. Antibiotics should be given for a minimum of 10 days and may need to be for longer periods Erythromycin 1mo-2yrs 125mg qds 2-8yrs 250mg qds 9yrs and above 250-500mg qds Flucloxacillin 1mo and above 12.5 25mg kg d in 4 divided doses Adults 250-500mg qds Swabs should be taken of eczema if infection is a recurrent problem skin and nose and possibly family members ; . Antibacterial antiseptics Oilatum Plus, Balneum Plus bath oils are useful as are also Flamazine or Dermol 500. Sodium fusidate containing preparations should not be used for more than two weeks because of increasing bacterial resistance and should only be used in combination with systemic antibiotics. Nystaform HC may be useful for the nappy area. Potassium permanganate is still useful in selected situations warn staining ; . Failure to respond to treatment Usually inadequate topical treatment check amounts prescribed and consider home visit ; but consider superadded infection with staphylococcus, streptococcus, scabies or herpes simplex. Bandaging Wet wraps, viscopaste or Zipzoc may be useful for lichenified eczema. Allergy Allergic contact dermatitis is rare in children but may need to be considered occasionally. Failure to respond to treatment may be an indication for patch testing in adults. Diet In general, dietary restriction is of little or no benefit in adults and in children. In infants under 1 year, it may be worth considering a dairy product and egg and nut free diet. It is advisable to avoid nuts below the age of 7yrs. RAST tests and prick tests are not particularly useful in identifying dietary allergens. Environment Although house dust mites may have an important role in atopic eczema, evidence for the benefits of eradicating mites is still limited. Simple measures to reduce house dust mite levels such as frequent vacuuming and the use of mattress covers may be of some use. Furry animals as pets should be discouraged.

The Fife Formulary and Abbreviated Drug list are in the process of being printed and should be distributed later this month. The format of the formulary has been changed to an A4 format to allow for the inclusion of prescribing guidance as it is distributed. There will not longer be an annual reprint of the formulary and the formulary will be updated through section reprints which will be distributed to all formulary holders. This update procedure will rely on the formulary holder replacing the old section with the updated section and we would advise annotating the inside of the section divider detailing the completion of the update. A complete list section updates and prescribing guidance issued will be available on the Area Drug and Therapeutics Committee website at show ot.nhs fifeadtc . The ADTC Bulletin will continue to provide bi-monthly information on formulary amendments and additions. The abbreviated drug list will be distributed to all Formulary holders to provide a desk top or pocket reference source. This will be produced annually to include all amendments and additions to the Fife Formulary, for instance, apo cloxi cloxacillin.

TionAN to provide individual insurance plans for their workers. In midOctober, former America Online chief Steve Case's venture, Revolution Health Group, bought twothirds of Extend. Pilzer spoke to IBD. IBD: Why should consumers opt to take more control of their medical care? Pilzer: Because unlike pensions, health benefits can be terminated at any time -- except ; union contracts. Companies are prohibited from eliminating pensions, even if they go through bankruptcy. Not the case with health benefits. IBD: Why should investors care and what can they do? Pilzer: Most companies with union employee contracts like General MotorsGM have retiree health benefits to pay, but I can't stress enough that these are open-ended obligations. Who knows what new devices will be invented tomorrow and that you, the investor, will become obligated to pay for? Our medical industry is constantly defining new diseases that weren't considered ; diseases a few years ago and coming up with new treatments. Yesterday's cause of death is today's cause of disability. People used to die of strokes, but today strokes only disable them. This trend is good if you're the person who doesn't want to die, but it's bankrupting many of our largest and oldest corporations. I'd encourage every analyst worth his salt to state the retiree health obligation of every company he follows. Investors need to be as wellversed about a company's health care obligations as he is about the company's business. I'll take my chances when I invest that a company will make a good widget, but I'd rather not take my chances that a company's retiree, because cloxacillinn benzathine.
