ARCHIE van Reenen and Allister Graham write: With reference to the article `State hospitals to be re-named': I would like to challenge the General Manager of District Hospitals, Dr Jabu Dlamini on his announcement that the names of institutions that reflect colonial and apartheid past must be removed forthwith. I take up the cudgels for just two of the institutions that he informs us are to be stripped of their names because of their colonial and appartheid past and probably will now be saddled with some unpronounceable chiefs name who lived in the area at some time, and who certainly made no financial contribution to the building and upkeep of these hospitals. Those of us who have lived in the district for fifty years or so, will remember when Elim Hospital was the only place to be when you were seriously ill, and that Dr and Madam Rosse were synonomous with good nursing and speedy recovery! They of course were supported and funded by the Swiss Mission and had definitely no ties or association with the `old regime'. So now this Doctor Dlamini and his committee are wanting to adopt this attitude of replacing historical names with political ones. The Van Velden Hospital is even more of an example of a political blunder and if the name has to be changed, there are all sorts of financial repercussions which come with the equation. Perhaps Dr Dlamini should be made aware of this. The fact of the matter is that Colonel Van Velden and his wife had a substantial estate which they wished to be used for the benefit of the whole Letaba district. A hospital had been suggested as a suitable project but the Van Veldens insisted that they were only prepared to fund the project if it was for the benefit of all inhabitants of the district which of course included both black and white sections of the community. The National Party who were in power at the time would not sanction this however and the whole business dragged on for the next three years. The final solution was achieved when the Van Velden Memorial Hospital, funded by the estate, was to be for white patients and the Letaba Hospital, funded by the Van Velden Estate and the Provincial Administration on a pound for pound basis, was started and then subsequently completed by the Provincial Administration and the Nederduits Gereformeerde Kerk. However, clinicians and patients must decide collaboratively whether to initiate maintenance treatment or provide more diligent monitoring with no treatment until a need is established by the development of a new episode. If a new episode develops when the patient is free of treatment, early intervention shortens the length of the new episode. 5 Patients who still have residual symptoms when medication is discontinued have a much greater chance of relapse when compared to patients that do not have any residual symptoms. Strict management regimes are however essential and regular follow up visits to the doctor mandatory.

A database search using three-dimensional-PSSM [45] and an ORF6 search-sequence identified more than 140 potential OATs. Figure 8 shows a sequence alignment of ORF6 with some of the better-characterized examples. The conserved autoproteolysis motif of the OATs was just one part of the generally high level of similarity throughout the length of these sequences. As mentioned above, the region containing Thr-111 and Gly-112, that may be involved in oxyanion stabilization, is highly conserved, as are the C-terminal residues. Furthermore, Lys-170, which may bind the substrate -carboxylate, is strictly conserved. The activesite residues that may bind the substrate side-chains Glu-260, Asn-388 and Arg-305 ; are also conserved, supporting their proposed role. It is interesting that the recently identified ORF6 homologue encoded by oat1 from S. clavuligerus, that is clustered with homologues of other clavulanic acid biosynthesis genes orf2orf4 ; , [18] differs in key regions of the amino acid sequence compared with ORF6. OAT1 possesses a modified autoproteolysis motif Figure 8 ; . However, due to the conservative substitution of serine for threonine, autoproteolysis may still occur. The proposed acetyl-oxygen-binding residues are conserved in OAT1, as is Lys-170, but the proposed substrate side-chain-binding residues are not. The three C-terminal residues of ORF6 are also not conserved in OAT1 although this might not prevent their functioning in a similar role. Taken together, these observations suggest that OAT1 is either an unusual OAT or that it has an alternative function, possibly directly associated with clavam biosynthesis. Since the introduction of lactam antibiotics, bac terial resistance to these agents has become an increasing problem. The production of the enzyme lactamase by both Gram positive and Gram negative bacteria is the most common mechanism of resistance to lactam antibiotics [1]. Lactamases hydrolyze the amide bond of the lactam antibiotics to create an ineffective antimi crobial agent; the enzymes have been grouped into four classes A, B, C, and D ; based on amino acid sequence homology [2, 3]. The most prevalent plasmid mediated lactamase in Gram negative bacteria is the TEM 1 lactamase from class A [4]. Like other class A lacta mases, TEM 1 lactamase is capable of hydrolyzing both penicillins and cephalosporins [2]. To overcome the drug resistance mediated by TEM 1 lactamase, extended spectrum antibiotics including aztreonam, cefotaxime, and ceftazidime were developed. However, soon after their introduction, selective pressure resulted in the emergence of variant lactamases capable of hydrolyzing these antibiotics [5]. An additional strategy that has been employed to combat antimicrobial drug resistance is the use of lac tamase inhibitors such as calvulanic acid, sulbactam, and tazobactam. Although not capable of antimicrobial activ ity themselves, these agents are used in conjunction with and acenocoumarol and clavulanic.

Antimicrobial susceptibility and -lactamase activity Table 1 shows the susceptibilities of the eight B. fragilis strains analysed in this study to six -lactam antibiotics. B. fragilis strains AK-4 and 119 MIC, 16 mg l1 ; and AK-2 MIC, 8 mg l1 ; were the most resistant strains to imipenem. However, when we tested the activity of imipenem in the presence of 4 mg cllavulanic acid l1 , it was observed that the sensitivity of B. fragilis AK-4 increased substantially MIC, 0.5 mg l1 ; , but the change in strains 119 and AK-2 was relatively small MICs, 8 and 4 mg l1 , respectively ; . All of these strains were resistant to ckavulanic acid alone at a concentration higher than 128 mg l1 . As shown in Table 2, six of the eight strains produced -lactamases and the cfiA gene was present in strains 119, AK-2, 2013E and 7160. However, we were only able to detect metallo--lactamase activity for strains 119 and AK-2. Expression of the group 3a, class B metallo--lactamase encoded by the silent cfiA chromosomal gene required the location of an IS immediately upstream Arpin et al., 2002 ; . Edwards 1997 ; reported that various strains of B. fragilis that produced -lactamases were resistant to the action of benzylpenicillin and cefoxitin and displayed moderate resistance to imipenem. However, the -lactamases of these micro-organisms were unable to hydrolyse either cefoxitin or imipenem in vitro. Similar results were obtained in this study, as in strains 119, AK-2.

Numerous studies have shown that standarized extract of Ginkgo biloba EGb 761 ; has significant influence on neurotransmitter receptors of the central nervous system and can prevent age-related memory impairement. Increasing evidence suggests that activation of the N-methyl-D-asparatate NMDA ; receptor may play an important role in spatial learning and memory. Our earlier study showed that EGb 761 could attenuate spatial memory deficits and learning decline of rats resulting from administration of the NMDA receptor antagonist MK-801 ; [Blecharz-Klin et al., Pol Przegl Kardiol, 2004; Blecharz-Klin et al., Pol J Pharmacol, 2004]. The present study was undertaken to investigate the effect of standardized extract of Ginkgo biloba leaves and D-cycloserine a partial glycine binding site agonist of the NMDA receptor complex ; on the spatial navigation in aging rats. Eighteen months old Wistar rats received for three months EGb 761 in drinking water 150 mg kg day ; . Half an hour before the behavioral trials, rats received D-cycloserine 3 mg kg ip ; . Water maze procedure was carried out for 4 days by training rats using 4 training trials per day. Memory was assesed by a trial performed 24 h after the last training. Motor activity and motivation attitude was analyzed in the visible platform task. In the acquisition testing, no significant differences were noted between experimental groups [F 3, 28 ; 1.16, p 0.05]. In the probe trial on day 5, the group pretreated with EGb 761 150 mg kg day ; showed improved memory in crossings over the position of the platform in target quadrant compared to the control EGb 150: 7.00 1.34 ; Control: 3.25 1.2 ; and the group treated with D-cycloserine C: 2.88 0.83, p 0.05 ; . EGb 761 and D-cycloserine administered concurrently were less effective EGb 150 + C 4.38 1.61 ; . Our results contribute to better elucidation of EGb 761 beneficial effects on aging-related neurodegenerative processes and could provide new perspectives in protection against age-associated mood and memory impairment.

Our ingelvac prrs business expanded significantly in the NAFTA region. The launch in Mexico exceeded all our expectations. In only 12 months ingelvac prrs mlv became the most important vaccine against this porcine illness, which is associated with high animal losses. The same positive trend was also registered in the USA and Canada, a development that underlines our leading position in this technically extremely challenging market segment. Cattle segment Solid growth in the core segment cattle proves that our product portfolio is on the right track. In addition to our traditional mastitis products, which performed well with their high market penetration, our vaccine range also posted excellent sales figures increasing by as much as 7 % in the NAFTA region. But the highlight of 2004 was our highly effective NSAID metacam which has become one of the world's most important anti-inflammatory and pain-relieving drugs. Its strong growth in the food-producing animal segment shows that farmers are attaching greater importance to the effective treatment of inflammation and alleviation of pain, and thus to the well-being of their animals. Companion animal segment In this segment, we have consistently consolidated our important role in many key countries. metacam has become an established product in the small animal segment, significantly enhancing the quality of life of dogs and cats with chronic illnesses, giving them a longer and, above all, more comfortable life. The positive business trend in the USA and Europe in particular exceeded expectations and ensured that. Eikenella corrodens is part of the normal human oral flora. Clinical infections include bites and abscesses in respiratory and GI tract. Eikenella corrodens is resistant to clindamycin, cloxacillin and metronidazole; and variable to first generation cephalosporins and erythromycin. E. coli may show variable susceptibility to the first generation cephalosporins. For wound, urine, other non-sterile body fluids isolates, both cefazolin and cephalexin are reported as the results may be different. For all other coliforms, the antibiotics are interchangeable. Rare strains of E. coli , Klebsiella pneumoniae and K. oxytoca have acquired a plasmid that renders them resistant to all cephalosporins including the 3rd generation cephalosporins and ceftazidime. This enzyme is called an "extended spectrum beta-lactamase" EBSL ; . Strains possessing these traits are serious nosocomial pathogens in some hospital units. These strains may also be multi resistant to quinolones, aminoglycosides and trimethoprim-sulfamethoxazole. Antibiotics of choice includes imipenem and meropenem for serious infections. Antibiotics including clavulanic acid combos such as timentin and clavulin may also be effective for some strains of ESBL.
