Ystic fibrosis CF ; is the most common lethal disorder in Caucasians 1 ; . The high morbidity and mortality in CF is due to chronic respiratory infections, culminating with chronic colonization with the bacterium Pseudomonas aeruginosa 2 ; . CF caused by mutations in the gene encoding a protein termed cystic fibrosis transmembrane conductance regulator CFTR ; . The exact connection between CFTR mutations, their physiological effects, and P. aeruginosa infections has not been fully established, as reflected in the multitude of proposals explaining the predilection to bacterial infection in CF 316 ; . The disease-causing CFTR mutations affect the processing, intracellular localization, and activity of this protein 1 ; . CFTR functions as a chloride channel 1 ; , but also has pleiotropic effects on the activity of other ion transporters including the amiloridesensitive epithelial sodium channel ENaC; refs. 17 and 18 ; . In attempts to explain the lung pathology in CF, several models have been proposed. It has been reported that defensins are impaired by high salt in CF lung secretions 4 ; . However, the electrolyte composition of the airway surface fluid may not to differ between normal and CF lungs 6, 19 ; . In contrast to the high-salt hypothesis, a low-volume proposal has been put forward in which uninhibited ENaC leads to hyperabsorption of sodium and water, leading to low airway surface liquid volume, which in turn impedes mucociliary clearance 7 ; . However, mucociliary clearance alone also cannot explain the predilection for P. aeruginosa, as there are substantial differences between CF and diseases such as primary ciliary dyskinesia 8 ; . Thus, the high-salt or low-volume proposals remain controversial 9 ; . In separate line of study, reduced inducible nitric oxide synthase iNOS ; production and NO output have been suggested to play a role in susceptibility to P. aeruginosa 10 ; . However, infection models in iNOS transgenic mice support only a limited role for NO in innate defenses against P. aeruginosa 11 ; . Recently, it has been proposed 14 ; that CFTR itself acts as a receptor for P. aeruginosa in bacterial uptake by the epithelial cells.

She was confident that no issues relating to Alvesco were left unresolved. Altana noted that the complainant alleged that an unnamed nurse informed him that a GP representative from Altana had told her that he endorsed Alvesco as an alternative to other steroids and to step 3 of the BTS Guidelines. Altana submitted that it had no more information on this meeting. The company did not have the nurse's name and it was therefore impossible for it to be certain that it had obtained the correct electronic meeting notes that were created. Although there was a short list of meetings that this representative undertook with nurses in the area between 2 June and 9 June, Altana was not able to use the electronic record to give it highly relevant information, which would have helped create a more robust version of events. However, on interview, the representative was extremely surprised to learn of the complaint, as he had not deviated from the agreed product messages or the communication from one of the representatives at the meeting with the complainant in any of the potential meetings from which the complaint arose. He was consistent with the primary care sales materials used provided ; . For further clarity during the interview, he was asked to state his understanding of the Alvesco positioning that had been endorsed by the complainant; he responded in line with both the Altana Alvesco positioning and the positioning supported by the complainant that Alvesco could be used once BDP had not been successful and before resorting to combination therapy. Altana submitted that the behaviour of its representatives had been of the highest order, promoting a product in line with the marketing authorization, the BTS guidelines, current medical practice and within the tight confines of the agreement with the complainant to use his name in support for a specific product positioning. Therefore Altana did not consider that it had breached Clauses 2, 9.1, 11.3 or 15.2 of the Code. The briefing notes and sales materials provided in its response unequivocally confirmed that Alvesco was positioned as step 2 therapy, in line with the BTS guidelines and current medical therapy. Whilst the complainant did not specifically cite any one particular piece of promotional material for censure Altana was certain that all of its materials were robust and complied with the Code. Therefore Altana did not consider that Clauses 7.2, 7.4 or 7.6 had been breached. In summary, whilst Altana deeply regretted that a misunderstanding occurred during a meeting between its representative and a nurse it could not be held responsible given the high standards of both the promotional positioning by the representative and the promotional materials and the agreement with the complainant to use his name during the call to support the positioning of the product. FURTHER COMMENTS FROM THE COMPLAINANT The complainant stated that he and his colleague were astonished by Altana's notes of the meeting as this was certainly not their recollection of how the.

The localization of amiloride-sensitive Na + channels in Na + -transporting epithelia was examined using antibodies made against amiloride-binding Na + channel protein purified from bovine kidney. The distribution of the channel protein was determined in thick frozen sections at the light-microscopic level using indirect immunofluorescence, and at the electron-microscopic level using immunogold labelling. In the cells of both the intact bovine collecting tubule and A6 confluent monolayers, only the luminal or apical-facing surface membranes showed staining. Sodium channel proein 'was characteristically localized on microvillar domains of the apical plasma membrane. Little or no basolateral membrane staining was evident. Channel protein was also absent from subapical vesicles and tight junctions, and was not found in bovine renal proximal tubules, cultured human secretory sweat coils, non-epithelial Chinese hamster ovary CHO ; cells or human skin fibroblasts.

DISCUSSION Furosemide produced increases in both pHi and Ca! + . Given the sensitivity of the apical potassium channels to pH1 and Ca!. + see Introduction ; it would appear that pH1 rather than Ca!j + is responsible for the observed increase in channel activity elicited by furosemide. However, it is also apparent that whilst the change in CaW + is not directly responsible for the upregulation of channel activity, it is necessary to give exchanger activation. As predicted by our initial hypothesis, the furosemide-induced alkalinization was sensitive to amiloride; this result is in keeping with the patch clamp data, where furosemide increased channel activity in an amiloridesensitive manner. Thus addition of furosemide activates the Na + -H + exchanger, resulting in intracellular alkalinization.

| Abstract--Vasopressin is known to acutely stimulate sodium transport in the renal collecting duct. We investigated the long-term regulation by vasopressin of the epithelial sodium channel ENaC ; in the rat kidney. Five-day infusion of dDAVP a V2 receptor agonist ; to Brattleboro rats lacking vasopressin induced a marked increase in - and -subunit ENaC mRNA levels in the renal cortex , 85%; , 100% ; , with no change in -ENaC mRNA. Expression of - and -ENaC mRNAs was also enhanced in lung , 49%; , 33% ; but not in distal colon an organ devoid of V2 receptors ; . Similar results were obtained in Sprague Dawley rats after either partial water restriction or dDAVP infusion for 5 days. Transepithelial voltage and transepithelial sodium and water net fluxes were measured in isolated perfused cortical collecting ducts of Brattleboro rats. Acute addition of 2 10 mol L dDAVP to the bath increased sodium and water fluxes in the same proportion, and to a far greater extent in dDAVP-infused than in control Brattleboro rats change in Na net flux, 337 30 versus 49 11 pmol min 1 mm 1, respectively; P 0.001 ; . These effects were abolished by amiloride. Extrarenal water losses, partly originating from the lung, were reduced by high plasma vasopressin level. This study shows that vasopressin increases sodium transport in the renal collecting duct and probably in the lung, through a differential transcriptional regulation of ENaC subunits. This effect is followed by isoosmotic water reabsorption and likely contributes to the process of water conservation. It could lead to less efficient sodium excretion, however, and thus participate in some forms of salt-sensitive hypertension. Hypertension. 2001; 38: 1143-1149. ; Key Words: kidney lung colon water transport sodium.
Given all our social injustice, all our desire for a better world, and all the shortcomings of the western medical tradition, the present is much better for most people because of our historical determination to blend science and human art and idealism in bettering our health. "Most of us, if pressed, would confess that our own lives have already been saved, or at least made more comfortable, by doctors, drugs, or both.
