Been under long debate. The catalytic mechanism of CGTases and a-amylases is thought to be similar to that proposed for hen egg white lysozyme. In this enzyme Glu35 acts as a proton donor while Asp52 stabilizes the oxocarbonium intermediate Blake et al., 1967 ; . For further details the reader is referred to Strynadka and James 1991 ; and Mooser 1993 ; . Using the ideas on the lysozyme mechanism and data from soaking experiments, Matsuura et al. 1984 ; proposed Glu230 Glu257 in CGTase numbering ; in TAKAamylase from Aspergillus oryzae to be the proton donor and Asp297 Asp 328 in CGTase ; the stabilizing acid. Later on Buisson et al. 1987 ; suggested on the basis of the 3D structure of pig pancreatic a-amylase that Asp197 and Asp300 were the catalytic residues Asp229 and Asp328 in CGTase numbering ; . The difficulties in assigning specific roles to these residues were mainly caused by their close proximity. Even the high-resolution structures of CGTase could not resolve this question Klein & Schulz, 1991; Lawson et al., 1994 ; . Recently Qian et al. 1994 ; concluded on the basis of the structure of a complex of pancreatic a-amylase PPA ; with an acarbose-likeinhibitor that Glu233 in mammalian a-amylase analogous to Glu257 in CGTase ; is the most appropriate candidate for the role of the general acid in the first stage of the catalytic process. Their inhibitor consisted of residues A, B, and C of acarbose see Figure 1 ; with a maltose linked through an a- 1-4 ; bond to the 0 4 atom of residue A. We will refer to this inhibitor as G5-acarbose. Our present results on binding of the inhibitor acarbose in the active site of CGTase further clarify the roles of the three carboxylates in the active site. In native CGTase a strong hydrogen bond 2.5 A ; exists between the carboxylic groups of Glu257 and Asp328 Klein & Schulz, 1991; Lawson et al., 1994 ; , indicating that one of the carboxylates is protonated, most probably Glu257 Lawson et al., 1994 ; . When acarbose is bound, Asp328 moves toward the inhibitor and hydrogen bonds are formed between the Asp328 side chain and the C2 and C3 hydroxyl groups of the B sugar residue Table 4 ; . Acarbose and hplcMaria L. Tupas, MD On Wednesday afternoon July 6, 2005, the AAP California Chapter 4 welcomed Congresswoman Loretta Sanchez and her staff to the chapter office in Garden Grove. Ms. Sanchez represents the 47th District of the U.S. which covers the cities of Santa Ana, Garden Grove, Fullerton, and Anaheim. One of her staff members, Khoi Ta, made contact with Dr. Quynh Kieu, past chapter president, and a practicing pediatrician in Fountain Valley, to arrange the meeting. Rep. Sanchez, a staunch supporter of issues dealing with minorities, wanted to learn more about Project Vietnam, a project of the International Child Health Committee of Chapter 4. Garden Grove in Orange County is home to the highest number of ethnic Vietnamese outside of Vietnam. Project Vietnam, began in 1996 under the auspices of the local AAP chapter, when a group of dedicated Vietnamese-American pediatricians recognized the dire need to improve the healthcare of children in poor rural communities in Vietnam. Since then, the medical mission trips held twice a year have provided reconstructive and ophthalmologic services, comprehensive medical examinations at villages and rural schools, and training for local health personnel. The credibility of the group allowed Project Vietnam to set up a series of pilot programs across the country. An important example was the Vitamin K Program for Newborns. Recognizing the prevalence of brain hemorrhages in newborns because of vitamin K deficiency, a condition which has disappeared in the US after the AAP recommendations for vitamin K injection after birth, Dr. Kieu urged a Vietnam study in 2000. It demonstrated an annual incidence of 12, 500 infants hospitalized for severe brain hemorrhages, with a mortality rate of 17.5%, and neurological damages in 40% of the babies. To overcome the reluctance of the government towards the administration of the vitamin K injection, Project Vietnam and the National Institute of Pediatrics-Hanoi, set up a pilot program in 2001, at the province of Ha Tay which had the highest number of affected babies. About 85, 000 newborn were protected with no occurrence of significant adverse effects. The practice of neonatal injections of Vitamin K was adopted as a national policy in 2003. Proj. Over two face to face meetings, drawing on their own clinical experience using existing guidelines, and government reports which focussed on issues specific to diabetes and related disease areas and or to the elderly, the Steering Committee identified a number of research questions to guide the literature searches. These questions centered attention on developing clinically relevant recommendations about aspects of care which may require different approaches in the elderly compared to younger adults with diabetes. They are: 1. Is case detection and diagnosis for Type 2 diabetes in the elderly worthwhile? 2. What clinical and laboratory assessments should be recommended for elderly people with diabetes and are there differences in treatment targets for the elderly? 3. Are there special considerations for elderly people with diabetes with regard to hyperglycaemia? 6. Are primary prevention strategies for Type 2 diabetes effective in the elderly?, because glucovance. Analysis of the hydrogen bonding network Table 4 ; shows that this residue makes the largest number of hydrogen bonds with the nearby active site residues, including two of the three catalytic residues Glu257, Asp328 ; and the very conserved His327 residue Svensson, 1988 ; . The OE2 atom of Glu257 side chain is at 2.9 A from the scissile bond oxygen strongly suggesting that this atom is protonated. Further binding interactions of the sugar ring come from a stacking contact with the aromatic ring of TyrlOO which is approximately and amiloride and acarbose.
